This webinar, Part 1 of a 2-part series, will feature Marie Abraham from the Institute for Patient and Family Centered Care.  During this first webinar, Marie will discuss how practices can implement, sustain, and retain patient advisors. A significant amount of time for this webinar will also be devoted to Q & A and troubleshooting/brainstorming discussions between Marie and practice teams to help teams develop action steps towards improving their patient engagement/advisory work.

During this month's Patient Centered Medical Home/Health Home webinar, we will explore one aspect of behavioral health integration that involves motivating patients to change their behaviors in favor of more healthy lifestyles.  Tom Bartol, a Family Nurse Practitioner in Maine, will discuss how he has changed his practice to better address patient's behavioral health and, in turn, their overall health and health care.  He will provide practical tips and tools for other primary care providers and team members on how to address patients' behavioral health needs and the impact behavioral - physical health integration will have on patients and practice teams alike.

Health care transformation has begun to identify the need to create solutions withcommunity members, not for them. There is increased recognition that the traditional professional expert and provider/consumer models are inadequate for solving many of the problems confronting us.

This session will focus on the emerging role of the “citizen professional” who works alongside other citizens to co-create new ways to address health care challenges. You will learn about a field-tested process called Citizen Health Care for constructing non-hierarchical working groups of professionals and other citizens. You will hear from an activated citizen who is working on health care transformation in a number of arenas. Using the example of an initiative called Baby Boomers for Balanced Health Care, you will hear both academic and citizen perspectives and will garner new ideas and strategies. 


The Comprehensive Primary Care (CPC) initiative is a national Centers for Medicare & Medicaid Services initiative fostering collaboration between public and private health care payers to strengthen primary care. Medicare will work with commercial and State health insurance plans and offer bonus payments to primary care doctors who better coordinate care for their patients. Learn how the Health Collaborative, the Greater Cincinnati Health Council, and HealthBridge are working together to implement CPCi in Southwest Ohio and Northern Kentucky.

This webinar is presented by Richard Shonk, MD, PhD. Dr. Shonk is Chief Medical Officer of the Greater Cincinnati Health Council, the Health Collaborative, and Healthbridge, nonprofit healthcare improvement organizations. He was previously the Market Medical Director for United HealthCare. He is a member of the Governor’s Task Force in Ohio for Patient Centered Medical Home. He is a founding member of the Ohio Patient Safety Institute and served as chariman of its board. Dr. Shonk is a board-certified physician in Family Medicine and a Certified Physician Executive with the American College of Physician Executives. He earned doctorates in Medicine and Pharmacology from Case Western Reserve University in Cleveland and attended Ohio Dominican College in Columbus.
