The need for collaboration between primary care and public health is widely recognized, but these sectors don’t have a lot of experience working together. To address this, the de Beaumont Foundation, Duke Community and Family Medicine, and the Centers for Disease Control and Prevention have developed a new tool – A Practical Playbook: Public Health & Primary Care Together. The Practical Playbookis a free, web-based tool designed to facilitate public health and primary care integration by offering a variety of resources for primary care providers and public health officials.
In this webinar, we explore the new tool and how it can support the implementation of community projects that aim to improve health and reduce health related costs.
